Please fill out this form if you are a new client or if you have not been to the clinic in more than 3 years. This is not for appointment requests. Please call if you are already established with us and need to make an appointment.
NOTICE: ALL new clients will be required to fill out our new client registration form and place a $60 non-refundable deposit to hold their appointment slot. New client deposits must be used within 30 days of the deposit being placed. Multiple appointments require multiple deposits that will go towards your visit if used within 30 days.
This is to ensure our availability and our patient’s commitment. You will NOT have to put deposits down for future appointments once your pet is established, but you will be charged a $75 missed appointment fee if you do not give a 24-hour advance notice to cancel your appointments in the future.
We will call you to schedule an appointment as soon as we have one available. If we do not have an appointment available in regard to your pet’s urgent needs, we will notify you. Please fill out everything as accurately as possible!
